The visit chooser and checklist run in your browser. They do not ask for names, passport numbers, symptoms or medical records.

Local selections

Your city and phone choices stay in page memory and reset on reload. This application sets no cookies and uses no local storage or third-party analytics. Your city and phone choices are not transmitted. When usage measurement is enabled, this site receives a broad page category, a coarse source category or an approved campaign label, a fresh random identifier for this page load, and fixed events such as opening an official source or copying a checklist. It does not receive your checklist, chosen city, phone condition, hospital selection, full URL or referrer. The random identifier stays only in page memory and temporary server records. It is not stored in browser cookies or local storage and does not identify a person. Temporary records older than 24 hours and aggregate counts older than 90 days are removed on the next accepted usage event. The application honors Global Privacy Control and Do Not Track by skipping usage collection. Reloads and navigation count as new page visits; the counts are not unique users or patients. Hosting infrastructure can process connection information separately.

Optional feedback

When the feedback form is available, pressing Send feedback transmits only a fixed response and a broad page category to this site. The application stores daily aggregate vote counts, not names, medical details, addresses, IP addresses or individual response records. Votes are not unique people. Counts older than 90 days are removed when the next feedback is accepted. Hosting infrastructure may process connection information separately. Feedback is not sent unless you submit it.

Copy and print

Copy uses your device clipboard when you request it. Print opens your browser’s print controls. How your device stores, prints or syncs that content is outside this application.

External sources

Opening a source takes you to another website with its own practices. Links suppress referrer transmission; this application does not send your checklist to hospital websites.

Hosting

Cloudflare Workers serves this site and may process connection metadata such as IP addresses in its infrastructure. When optional feedback or usage collection is enabled, Cloudflare D1 stores the limited application records described above. These are first-party services, not a medical-record system. See Cloudflare’s Privacy Policy. No medical or identity data should be sent to this site.

Correction emails

Emailing us sends your address, message and any included attachments to the operator. The correction service uses Cloudflare Email Routing, an operator-controlled Agentic Inbox on Cloudflare Workers and Durable Objects, R2 for attachments, and forwarding to the operator’s Gmail inbox. We use these messages to investigate public-source corrections or privacy requests. Email records remain until the operator deletes them; there is no verified automatic email-retention timer. Deletion requests need to cover both inbox copies and any stored attachments; provider backups follow their own policies. Do not send health records, passport details or attachments. This service is separate from the anonymous fixed-choice feedback form.
